➤Regarding the Pension Refund, take the following steps:
①YOU submit a move-out notification at the city hall and YOU request a residence certificate住民票 with *individual number.
*Verification of individual number may be required when you apply for the Income Tax Return later.
②YOU mail the required documents to Japan Pension Service.
Required documents:
a. Application for the Lump-sum Withdrawal Payments Form

b. A copy of your passport (pages including your name, date of birth, nationality, signature, status of residence)
c. A document verifying the bank name, branch name and address, account number, and the name registered as the account holder. (A copy of the bank account information card is preferable)
*A bank verification stamp is needed for the application.
d. The individual’s Pension Handbook/ 年金手張 (blue booklet)
**If you submit an application in the mail, the mail must arrive at Japan Pension Service on or after the (planned) deletion date of your residence record.

➤Regarding the Income Tax Return, take the following steps:
①YOU appoint your Tax Representative, who has to reside in Japan.
②YOU visit Toyono tax office to register your Tax Rep. by submitting the required documents.
Required documents:
a. Notification of Tax Rep.
*Fill out Tax Rep.’s information.
*Prepare 2 copies, obtain an acceptance stamp on each copy, and submit one, receive the other for your record.
b. A residence certificate with individual number.
c. Residence card for identification.

③YOU leave Japan.
After 4-5 months.
Japan Pension Service will transfer funds to your bank account and YOU will receive “Notice on Payment of the Lump-sum Withdrawal Payment” at your post-Japan address.
④YOU send the original copy of “Notice on Payment of the Lump-sum Withdrawal Payment” to your Tax Rep. in Japan.
⑤TAX REP. will visit Toyono tax office and submit the required documents in person.
Required documents:
a. Final Tax Return Form B page 1, 2, and 3.
*It is advisable for Tax Rep. to fill out at the tax office while being instructed by a tax office official since the form is complicated.
b. The original copy of “Notice on Payment of the Lump-sum Withdrawal Payment”
c. Tax Rep.’s identification document (Driver's license in Japan or Residence card)
d. Tax Rep.’s bank account information
⑥Income tax will be refunded to Tax Rep.’s bank account.
⑦TAX REP. will remit the tax refund to you.
You don’t need to fill it out. The coverage history is needed for a person who doesn’t know her/ his basic pension number.
